    • 1). Obtain a copy of the incorrect birth certificate. Contact the hospital where the person was born to discuss the error. Request written documentation and records from the hospital to confirm the mistake. Obtain certification of the error from the hospital in the form of written affidavits of those persons with knowledge of the error.

    • 2). Check with the court in the jurisdiction to determine exactly what documentation is required to be filed with the court. Generally, the court requires the filing of a petition with a supporting motion and documentation to request the court make the change of birth legal.
