- 1). Compile a list of fields you want to have in the application form. Examples of fields could include name, date of birth, or occupation.
- 2). Launch your word processing software and create a new document.
- 3). Type the label for the first field into your document. For example, you might want the first field to be labeled "Name."
- 4). Insert a table into the document, right underneath the label for the first field. The table should be as wide as the page, have only one row and one column, and have a solid black border. This is the open field to be filled in by respondents. Repeat this process until you have created all the fields.
