    • 1). Contact your state Medicaid office to obtain the necessary billing claim forms. Each state has its own unique Medicaid claim forms that must be submitted for reimbursement.

    • 2). Review state requirements for what can be billed under nonemergency transportation. For example, Indiana Medicaid allows Medicaid billing if the transportation is to and from other Medicaid services. Up to 20 one-way trips less than 50 miles may be billed for a single individual per year. Other states have different requirements.

    • 3). Complete claim forms. Claim forms will require the patient's full name, address, Medicaid number, Social Security number, date of service and a billing code for service(s).
