How Do I Get a Copy of My Certificate of Live Birth From Alaska?
- 1). Scan your driver's license and enlarge it. Print out the license or make a copy of it. Sign your full name on the same sheet of paper that the license is copied onto.
- 2). Download the Alaska Birth Certificate Request form (see Resources). Print the form and fill it out completely.You'll need your social security number, place of birth and mother's maiden name when completing the application.
- 3). Choose your form of payment. If paying by check or money order, make it out to the Bureau of Vital Statistics. As of June 2010, the required fee was $20. You also can pay by credit or debit card. To do so, simply input your credit or debit card number into the appropriate box on the application. If you want expedited handling, place a check mark in the appropriate box on the application. As of June 2010, expedited handling is $11.
- 4). Mail the completed form, photocopy of your license, and the fee to:
