How to Obtain a North Dakota Birth Certificate

    Order Onlne

    • 1). Log on to the online application for birth certificates (see Resources).

    • 2). Enter your email address or phone number in the field provided. Enter a password in the field provided. These will be used to verify your identity when you purchase the certificate. Click "Order Certificate Now."

    • 3). Click "Add Birth Certificate." Enter the required information about the birth certificate you are requesting. Click "Update."

    • 4). Click "Checkout." Enter your billing address and shipping address information. Select the shipping method you prefer. There is no charge for First Class Mail.

    • 5). Enter your credit card information and click "Submit." Your order will be processed. Because you are still required to submit a photocopy of a photo ID, you'll be provided with an email address and fax number to which you can send it.

    Order by Fax

    • 1). Download the request application (see Resources).

    • 2). Complete the form with the birth certificate information, mailing address and credit card information. A credit card payment is required for the fax method. You can either submit a photocopy of photo ID via fax or have the application notarized.

    • 3). Fax the completed form to (701) 328-1850.

    Order by Mail

    • 1). Download the request application (see Resources).

    • 2). Complete and print out the form (same as step 2 from above).

    • 3). Write a check or money order for $7 to ND Department of Health. If you want to use a credit card, include your credit card information on the form.

    • 4). Mail the completed form to:

      Bismarck, ND 58505-0200

    Order in Person

    • 1). Go to the Vital Records Office in the State Capitol Building in Bismarck. The office is located in room 118 of the Judicial Wing. It is open between 7:30 a.m. and 5 p.m. Monday through Friday as of June 2010.

    • 2). Fill out the request application at the office and show your photo identification.

    • 3). Pay the required $7 fee. You should be able to obtain your certified copy within a few minutes.
