How to Change an Infant's Name on a Louisiana Birth Certificate
- 1). Complete the Application to Amend Certificate of Birth, available on the state's Department of Health and Hospitals website (see Resources). Complete the biographical information, including your name, address and phone number. Provide your infant's current name in Part I of the application. Also, include her date of birth, place of birth, sex and parents' names. List the name change in Part III of the form, noting the current name and new name.
- 2). Obtain evidentiary documents to support your name change. Louisiana allows several documents for a name change for a child under age 12, and the type of document you choose depends on the circumstances surrounding the name change. For example, if you are changing a last name, an acknowledgement of paternity can support your change request. Other accepted documents include a name-change judgment, hospital record, baptismal certificate or affidavit executed by both parents.
- 3). Make a photocopy of the current birth certificate to include with your application.
- 4). Submit your application packet to the Louisiana Vital Records Registry. This packet should include your application, a copy of the current birth certificate and your evidentiary document. As of April 2011, the state's fee for processing this application was $18, as well as an additional $9 for a copy of the amended birth certificate. Include payment with your application.
