How to Get a Birth Certificate in Columbia, Missouri
- 1). Order your birth certificate online from VitalChek (see Resources). Some of the information required includes your current name, place of birth, date of birth, name at birth and parents' names. You must also provide your reason for requesting the birth certificate and your mailing address. It will take 3 to 5 days to process your order before it is shipped. It costs $15 to order the first copy of your certificate and VitalChek will include a $9.95 processing fee. Pay for your order with a debit or credit card.
- 2). Call VitalChek at 1-877-817-7363 to place your order over the phone. You will have to provide the required information to an agent and make a payment for the service with a debit or credit card. The service line is available for 24 hours daily.
- 3). Obtain your birth certificate from the local health office. Download an application from the website for the Columbia-Boone County Department of Health (see Resources). Complete the form and take it to the Department at the following address:
