How do I Apply for KidsCare Medical Insurance in Tucson, AZ?
- 1). Download, print out and complete the Application for Assistance from Arizona's Department of Economic Security website. You will need Microsoft Word to access the application. You can also call 602-417-7000 and request that a paper application be mailed to you. If you prefer to complete an online application rather than a paper application, go to Health-e-Arizona, the online application system for AHCCCS. Create an account, follow the prompts and fill out and submit your application online.

Gather the following documentation and information so that KidsCare can verify the information you put down on your application: Social Security number; birth certificate; proof of U.S. citizenship or other legal immigration status; proof of identity; proof of income; proof of household, medical and childcare expenses; bank account statements; savings bonds, life insurance and retirement plan statements; and proof of health insurance, if applicable. You must provide supporting documentation for each person you are applying for.
- 3). Find your local FAA office. Navigate to the online office locator tool on Arizona's official FAA website; select "Tucson" from the pull-down menu and hit "submit."
