Where to Go

• If you were born in Utah County, you can get a copy of your birth certificate through the Utah County Health Department's Office of Vital Records. Those born in the state of Utah but in another county after 1951 also can go to the county health department; those born before 1951 can go to the health department, but it must be obtained through the other county. At the state's Office of Vital Records and Statistics, you can get a copy of the certificate for any birth that occurred in Utah since 1905.
  
  

What You Need

• You will need to show current picture identification, such as a driver's license, state-issued ID, school or employment ID or passport. You can get a copy of the birth certificate for yourself or for an immediate family member. The cost of getting the certificate is $18 as of October 2010.
  
  

How to Get a Copy

• You can make a request to the county by calling (801) 851-7005 or by mailing a completed request form and check or money order to the Utah County Health Department Office of Vital Records. If you mail the request, include an enlarged, easily identifiable copy of the front and back of your ID.
