For example, the applicant may enter "Self," "Mother" or "Father." In addition, the applicant will need to sign for the certificate by typing in the designated field. Press "Continue."

    • 8). Choose the gender of the person on the certificate from the drop-down menu and select whether the person named is still living. Press "Continue."

    • 9). Enter the full names of the parents' of the person named on the birth certificate. Make sure to include the mother's maiden name, not married name. "Press Continue."

      Select the shipping method. Applicants can have the record expedited and it will arrive by UPS Air in one to two business days. The fee is an additional $18 (as of 2010). There is no additional fee for regular shipping. Below the shipping method enter the mailing address, email address and phone number in the corresponding fields. Press "Continue."
