    • 1). Notify your bank as soon as possible about the withdrawals in question. Call your bank (either your local branch or a toll-free number, if provided) so that they can deactivate your ATM card.

    • 2). Get a written statement showing the date and the amount of the illegal withdrawals. You can use your monthly statement or you can access your account online and print out your recent account activity. Take your statement to the bank, or have the paperwork ready if you are reporting the fraud by phone.

    • 3). Ask to speak to a manager -- if reporting the fraud in person -- who likely will have more authority than tellers. A teller likely will refer you to a manager anyway in cases of suspected theft or fraud.

    • 4). Explain the situation to the bank manager. Show the manager the transactions in question on your statement; she can access it electronically if you are speaking by phone.

    • 5). Answer the manager's questions. He will need to know if you still have your ATM card, if anyone else has access to the account and if there are any circumstances where someone could have seen your PIN (personal identification number).
