- 1). Find your original title. If you have lost or misplaced your title, visit the Tempe Department of Motor Vehicles and fill out a request for a duplicate title. As of 2011, it cost $4 to order a duplicate copy of the title. It takes three to four weeks to receive your duplicate title.
- 2). Detach the transfer of title and notice of sale from the title.
- 3). List the name and address of the charity you are donating the vehicle to on the transfer of title and notice of sale forms. List your name and address on the transfer of title form as well.
- 4). Check your exact mileage. List it on the transfer of title form.
- 5). Arrange for the pick up of your vehicle or bring it to the donation center. Go to the Tempe DMV with a representative from the charity. You both must sign the form in front of a DMV employee so the employee can notarize it.
- 6). Give the notice of sale form to the DMV to release yourself from liability connected to the vehicle.
- 7). Check your mail for a tax receipt from the charity you donated your vehicle to. Use the amount shown on the receipt as your deduction when you file your taxes.
