Fill out all the required paperwork at the DMV. This paperwork consists of a form that will ask you why you need a new copy of your driver's license or picture ID. You will check that you have had a name change on the form. You will also put all your personal identification information on the form such as your address, date of birth and Social Security number. Alternatively, you can download the paperwork to change your name from your State's Department of Motor Vehicle website, fill the paperwork out at home and bring it in. - 4). Get in line and hand over the paperwork you filled out and the documentation that you brought with you to the DMV clerks. They will make copies and give you back your documentation. Pay the fee, and then they will take a new picture of you. You normally will get a temporary copy of your identification card and they will send the normal picture ID in the mail within 60 days.
