    • 1). Wait until you are at least 19 years old and have lived in British Columbia for at least three months before you apply to change your name.

    • 2). Collect additional documents that you will need for your application. These include your birth certificate, any immigration documents if you were born outside of Canada, and a marriage certificate.

    • 3). Request a Change of Name application package from a Vital Statistics Agency or any government office. You can order the package via telephone by calling 250-952-2681. You can also request an application from the Vital Statistics Agency’s website (see Resources).

    • 4). Fill out the application and take it to your local police department office. You must have your fingerprints taken at the police department office. When the fingerprints are ready, the officer will seal them with your application and application fee and send the entire package to the Vital Statistics Agency in Victoria. The application fee is $137 for each adult name change and $27 for each additional minor child name change.

    • 5). Begin using your new name and applying for new identification cards as soon as you receive your Certificate of Change of Name in the mail.
