    • 1). Contact your financial institution to obtain a Written Statement of Unauthorized ACH Debit Form. Print in legible blue or black ink, read every section carefully, and complete all sections.

    • 2). Place your name on the line as the person who has confirmed that an ACH debit has happened on your account. Write in the originator, who is the merchant that took out the transaction. You need to include the account number involved with the disputed transaction.

    • 3). Indicate whether the ACH debit was not authorized by you for the account or whether the ACH debit was authorized but by a different amount than what shows on your account. Check the section part if you had authorized the ACH debit but it was revoked while following the payee company's agreement guidelines before the date when the debit became posted on your account. (In cases of revocation, you need to supply your financial institution with a copy of your Revocation and signed Stop Payment form within 14 days, or the financial institution will uphold transaction debits made to your account.)

    • 4). Mark the appropriate area if your converted check was improper. You can stop transactions from your account if a check entry and an ACH debit became processed for simultaneous payments. You can protest improper converted check transactions for not receiving a notice that the check may be converted into an ACH debit or you requested not to have the check converted. Also, you can stop unauthorized transactions if the check converted into an ACH debit had an altered amount from the written check; or a fake and unauthorized signature had signed for the item related to the ACH debit.

    • 5). List all the unauthorized transactions made to your account and include the dates the transactions happened. Print the name of the primary account holder along with the signature and date. Include the printed name, signature and date of the joint account holder. Send the Written Statement of Unauthorized ACH Debit Form to your financial institution.
