How to Obtain a 2005 W-2
- 1). Contact the payroll department of the employer that issued the 2005 W-2 and request a copy. Employers are required by law to keep copies of W-2 forms for at least four years. Avoid unnecessary delays by confirming and/or updating your current mailing address. If the company is out of business or doesn't have your W-2 on file, you can contact the IRS to receive a copy.
- 2). Call the IRS at 800-829-1040 and order a "Request for Copy of Tax Return" form, also known as Form 4506, or download it online for free (see Resources section).
- 3). Fill out the form completely and legibly. Enter the tax year 2005 on line 7 and sign your name at the bottom of the form on the signature line.
- 4). Mail in the completed tax request form with a check or money order in the amount of $57, payable to the United States Treasury to the address listed on the instructions for your state. Write your Social Security number and "Form 4506 request" on the check or money order.
- 5). Wait up to 60 calendar days for the IRS to process your request.
