How to Request a California Birth Certificate
- 1). Visit the California Department of Public Health website at cdph.ca.gov and click on "Birth, Death and Marriage Certificates" on the left side of the page.
- 2). Scroll down to the list of vital records and click on "Birth and Death Records." Go to step three on the new page and click on application for a certified copy of your birth certificate. Fill out the application completely and print it.
- 3). Take the sworn statement to a notary public and have it signed. If the birth certificate you are requesting is solely for genealogical research, the sworn statement isn't needed.
- 4). Make a check or money order payable to the Office of Vital Records in the amount of $14 per copy requested.
- 5). Enclose the application, notarized sworn statement and check or money order in an envelope and mail to:
