How to Obtain My Georgia Birth Certificate
- 1). Find out the name of the Georgia county in which you were born. You should also know your parent's full names, including your mother's maiden name.
- 2). Navigate to the Georgia Birth Certificate Request Form web page and fill out the form online.
- 3). Click on the "Generate Form" icon, and print out the page that appears. This is your birth certificate request form.
- 4). Sign the form after verifying that all the information you provided is true. Remember that a false statement, particularly with respect to your identity, could subject you to criminal prosecution.
- 5). Photocopy your driver's license, government-issued personal ID or employee ID, and include the photocopy with your application.
- 6). Include a money order or a certified check made out to Vital Records Services. A personal check will not be accepted, but you may pay by credit card if you decide to apply through VitalChek instead (see Resources). The fee is $10 for the first copy and $5.00 for each additional copy.
- 7). Mail the application, photocopy and payment to Vital Records, 2600 Skyland Drive NE, Atlanta, GA 30319-3640. Your birth certificate will be mailed to the mailing address that you provided on your application form. You should receive your birth certificate within a week after you mail your application.
