How to Obtain a Death Certificate in Los Angeles
- 1). Download and fill out the "Application for Death Record" and "Certificate of Identity" forms. Be sure to have the Certificate of Identity form notarized and sign both documents.
- 2). Enclose the two forms, check or money order for $12 and the self-addressed stamped envelope in a large envelop.
- 3). Mail the envelope to:

Birth, Death and Marriage Records Section

Allow 20 days to receive the death certificate.
