How to Change a Name Legally in Baltimore
- 1). Contact the Baltimore County Circuit Court at 401 Bosley Avenue in Towson, Maryland, or go online to the Maryland Judiciary site where you will find domestic relations forms to copy.
- 2). Complete the Petition for Change of Name form, sign and attach your birth certificate. File and pay a fee.
- 3). Arrange for a Publication of Notice in the newspaper. Ask the clerk for assistance with this procedure. The newspaper will send a confirmation note that they have published your information; make certain that the clerk has also received this notice. The judge will now consider your petition and any objections. He will then sign and send you the Order for Name Change.
- 4). Change your name with the Department of Motor Vehicles, Social Security Administration, Bureau of Vital Statistics, passport office, any creditors and the post office. You will need certified or original copies of the judge's order to show these agencies.
- 5). Tell employers, library, family and friends, banks, utilities, voting registrar, banks and schools of your name change.
