    Request a pre-1949 Death Certificate Online

    • 1). Log on to the Department of Records' website, http://www.nyc.gov/html/records/

    • 2). Click on the "Death Certificate" link under the "Vital Records" category in the navigation column on the left of the screen.

    • 3). Click on the "Request a Search and Certified Copy Online" link if you do not already have the death certificate's ID number. Or click on the "Request a Certified Copy Online" if you already have the certificate number.

    • 4). Complete the online form with the deceased's information. Click the "Add to Shopping Cart" button when finished. Note that each certified copy incurs a $15 fee, as of 2010.

    • 5). Review your order. Click "Continue Shopping" and repeat Steps 1 to 4 if you want to obtain additional death certificates, or click "Checkout" to complete your order.

    • 6). Enter your shipping information; there will be a fee for shipping and handling. Click "Continue."

    • 7). Enter your credit card information. Click "Continue."

    • 8). Review your order, fees, shipping address and credit card information. Check the box verifying the information and authorizing the charge to your credit card. Click "Continue."

    • 9). Complete the ordering process and wait four to six weeks to receive the certified death certificate in the mail.

    Request a Pre-1949 Death Certificate by Mail

    • 1). Perform Steps 1 and 2 as described above.

    • 2). Click on the "Request a Search and/or Certified Copy by Mail" link. An order form in PDF format will open.

    • 3). Print the order form, and enter the deceased's information by hand.

    • 4). Mail the form to the NYC Department of Records, 31 Chambers St., New York, NY 10007. Include a check for the appropriate fee(s) made out to the NYC Department of Records and a self-addressed, stamped envelope for the death certificate.

    Request a Post-1949 Death Certificate by Mail

    • 1). Log on to the NYC Department of Health and Mental Hygiene's Office of Vital Records website at http://www.nyc.gov/html/doh/html/vr/vrdappl.shtml.

    • 2). Click the in-text link "by mail." On the next screen, click the in-text link "application." An order form in PDF format will open.

    • 3). Print the order form, and complete the information required. If you are a blood relative (including spouse or domestic partner) of the deceased, include with the order a photocopy of your driver's license, passport or other approved ID. If you are not a blood relative, include a photocopy of your ID and a photocopy of the deceased's will, insurance policy or other approved document that establishes your relationship to the deceased.

    • 4). Mail the application, ID documents and a check or money order made payable to the NYC Department of Health and Mental Hygiene to 125 Worth St., CN-4, New York, NY 10013. Include a self-addressed, stamped envelope for the death certificate.

    Request a Post-1949 Death Certificate Online (Blood Relatives Only)

    • 1). Log on to the NYC Department of Health and Mental Hygiene's Office of Vital Records website at http://www.nyc.gov/vitalrecords. Note that only blood relatives (including spouses or domestic partners) of the deceased can order a death certificate online.

    • 2). Click on the in-text link "death" at the top of the page. The website for the third-party company VitalCheck opens. This company handles death certificate orders for the NYC Department of Health.

    • 3). Check the radio button marked "Death certificates" and click continue.

    • 4). Answer the questions that appear on successive screens, and click "continue" after each answer. You will need to provide the state, city, borough and purpose for obtaining the NYC death certificate.

    • 5). Review your order (including fees) and click "continue." On the following screens, enter your identity, deceased's identity, shipping and billing information. Complete your order and receive the certified death record in about 10 days via U.S. Postal Service (expedited shipping is available for an additional fee).
