How to Order a Death Certificate in San Francisco
- 1). Go in person to the Department of Public Health in San Francisco between 8 and 11:30 a.m. or 1 and 3:30 p.m.
- 2). Request a death certificate. You will be given an application.
- 3). Fill out the application. Include the place and time of the deceased's passing, as well as their name and birthday.
- 4). Take the application to the intake window and pay $12 by cash, money order or Visa.
- 5). Show your ID if you need a certified, authorized copy.

- 2). Fill out the application and send it, along with the fee, in either postal or bank money order.
- 3). Include a sworn, notarized statement that you are eligible to receive a certified, authorized copy if you need a certified, authorized copy.
